It requests a receipt notification for a qualifying paper filing. It does not apply for a benefit, grant immigration status or tell you that your application was approved.

- The main form number and its official filing instructions, so you can confirm it belongs at a USCIS Lockbox.

What happens after you prepare it?
Review and print the completed page. USCIS says to clip G-1145 to the first page of the immigration form. Its current mailing guide places any applicable payment form first, followed by G-1145, G-28 if applicable, the main form, supplements and evidence. Check the main form’s signatures, fees and current mailing address. Keep delivery tracking and the USCIS receipt. Downloading does not submit anything.
